​If requesting an absence, tardy, or early dismissal from any marching band rehearsal, performance, or event.

Required Forms

1. 2026-2027 Marching Band Registration (Due 6/19 digitally)

​2. Student Physical 435-a (Due 7/27 to Mr. McCutcheon)
* Physicals must be dated AFTER May 1, 2026. Students with expired physicals will not be permitted to participate in rehearsals or performances until a new physical is turned in. *

3. Permission to Travel Form (Due 7/27 to Mr. McCutcheon)

Required Fees

 1. MTLSD Annual Registration Fee (Due 7/27 through Arbiter website)
​                - $75 per student
                - $150 family cap
​
2. MLBB Annual Marching Band Membership Fee (Due 7/27)
                - $200 (first student)
                - $150 (each additional student in a family)
